Mission Statement
The focus of the D4D Health & Wellness Medical Center, “Diabetics 4 Diabetics,” is working
one on one with all patients to bring about control, whether in diabetes management or
other health issues. We combine Conventional and Naturopathic Medicine to provide our
patients with the best possible of both worlds. This program is designed to work in
conjunction with the patient's personal physician. The progress of you as a patient
will be provided to hime or her as quarterly updates. You will also have indefinite
group supports, advice, and guidance.
Program Description
All sessions are conducted in the privacy of our office. The first month in the program consists
of intense one-hour session each week.
Week One: During the first week session, the following client information is
documented to establish a baseline to measure client’s success by the end of the program.
- Your medical history of diabetes.
- Your lab tests (Note: If none is available, we will request them. We need a current Chemistry Panel, Lipid Profile, and A1c Hemoglobin measurement in order to establish a baseline.).
- Your Body Fat percentages versus Lean Body Mass to establish a baseline.
- Height, Weight, Blood Pressure to establish a baseline (if blood pressure is too high, we request an authorization slip from your physician to proceed with exercise program).
- Establish BMI (Body Mass Index) to establish a baseline.
- Establish BMR (Basal Metabolic Rate) to establish a baseline.
- Establish a diary for one week to review before setting a meal plan to follow.
You will also be given an orientation during the first week which includes the following topics:
- Understanding Carbs, Proteins, Fat and Water.
- Reading Nutrition Labels and food ingredients.
- Maintaining a DID (Daily Intake Diary) and its importance as a tool for maintaining control.
- Monitoring glucose tests, when to do them, and when to log them for review.
- Spending grocery money wisely.
- Using the D4D web site facilities.
Weeks Two through Five: During weeks two through five we will have weekly office visits. During these sessions we will:
- Concentrate on nutrition documentation, lessons, and Q&A.
- Establish an exercise program to fit any restrictions or limitations.
- Deal with stress.
- Examine facts and myths of fitness and nutrition.
- Discuss Diabetes complications.
Weeks Six through Fifty-Two: During weeks six through fifty-two we will meet monthly
(every four weeks) for followup evaluations. During these meetings we will:
- Evaluate and modify Daily Intake Diary.
- Evaluate and modify exercise program.
- Evaluate and discuss glucose tests.
- Thoroughly discuss both your strong and weak points with an emphasis on constructive feedback.
Quarterly Tests and Measurements:The following are performed every 12 weeks throughout the full term of program.
- Lab test evaluation and documentation
- BMI, BMR, LBM documentation and comparison followed by recommendations.
- Weight and blood pressure changes
- Discussion, changes, recommendations to insure continued progress and control.
End of the Program
At the end of the one-year program, we will provide you with a printout of your success in every aspect of the program, A1c measurements, labs, and overall program.
Copies will also be provide for your family doctor. You will be urged to continue calling
D4D and using the web site for continued advice and support.
Insurance Information
We accept all PPO insurance plans, some EPO insurance plans and all idemnity plans. We accept
some HMO insurance plans. Call and we will be happy to verify your insurance.
